Note: If you are under the age of 18 years old, you are NOT able to register online for the A Day in the Life Project photography events. Please contact us for further registration instructions prior to the day of the photography event.

We are currently taking online registration for photography participants for the A Day in the Life of North Sacramento on Friday, October 16, 2009.

IMPORTANT PARTICIPATION / MODEL RELEASE AGREEMENT. MUST READ.
By submitting this online registration form, I hereby give A Day in the Life Project, Inc., the right to use the images that I photograph during the A Day in the Life Project event for publicity, advertising, promotion, exhibition or other lawful purposes. Further, I agree that I understand that I am the legal owner of the negatives or original data files I produce and I am entitled to the copyright on the images produced during the A Day in the Life Project event. I am also stating that I am of full age and competent to agree to this release. I also agree that this release shall be binding on me and any legal representatives.

By submitting this online registration form, I am also agreeing to the following model release in the event I am photographed by myself or another photographer to include video and media during the A Day in the Life Project, Inc. event. I do hereby give A Day in the Life Project, Inc., its assigns, licensees, successors in interest, legal representative, and heirs the irrevocable right to use my name (or any fictional name), picture, portrait, or photograph in all forms and in all media and in all manners, without any restriction as to changes or alterations (including but not limited to composite, exhibition, or any other lawful purposes, and I waive any right to inspect or approve the photograph(s) or finished version(s) incorporating the photograph(s), including written copy that may be created and appear in connection therewith. I hereby release and agree to hold harmless the A Day in the Life Project, Inc., the photographers, its assigns, licensees, successors in interest, legal representatives and heirs, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of the photographs, or in any processing tending toward the completion of the finished product. I also agree that the Photographer owns the copyright in these photographs and I do hereby waive any claims I may have based on any usage of the photographs or works derived therefrom, including but not limited to claims for either invasion of privacy or libel. By submitting this form, I am stating that I am of full age and competent to agree to this release. I agree that this release will be binding on me, my legal representatives, heirs, and assigns. I have read this release and am fully familiar with its contents.
